We have partnered with United Way to provide disaster relief assistance for those affected by the tornadoes in Mineral Wells.

Step 1: Click the link below and complete the application as thoroughly as possible.

Step 2: Gather and have ready any receipts, repair estimates, insurance information, and other supporting documents.

Step 3: United Way will contact you to collect additional information and review your application. Once your application is reviewed, Mineral Wells Center of Life will receive your referral from United Way.

Step 4: Mineral Wells Center of Life will assess your needs and determine how we may be able to assist you. We will stay in contact with you throughout the process and keep you informed of any updates.
